{"id":226,"date":"2020-03-18T16:42:51","date_gmt":"2020-03-18T16:42:51","guid":{"rendered":"http:\/\/gmvemsc.org\/covid19\/?p=226"},"modified":"2020-04-06T14:32:20","modified_gmt":"2020-04-06T18:32:20","slug":"jitso-accompanying-patients-during-transport","status":"publish","type":"post","link":"https:\/\/gmvemsc.org\/covid19\/bulletins-and-jitsos\/jitso-accompanying-patients-during-transport\/","title":{"rendered":"JITSO &#8211; Accompanying Patients During Transport"},"content":{"rendered":"\n<p>Emergency medical\nservices are a component of <em>critical infrastructure<\/em>. To protect\npersonnel during the pandemic, the following Just in Time Standing Order\n(JITSO) is in effect throughout Greater Miami Valley EMS Council immediately.<\/p>\n\n\n\n<p>To ensure effective\npatient care and minimize cross-contamination of EMS units and hospitals, EMS\nunits will only allow family members or support persons to be transported with\nthe patient when it is essential to patient care.&nbsp; All other requests for\ncompanion transports should be denied at this time.&nbsp;&nbsp; <\/p>\n\n\n\n<ul><li><strong><em>Examples<\/em><\/strong> of companions who must be transported in EMS units include parents or guardians of minors; historians for time-critical diagnoses such as stroke; or translators when significant language barriers exist.&nbsp; <\/li><li>Companions who are demonstrating infectious symptoms such as fever, cough, illness should not be transported (unless being treated as an additional patient in the patient compartment).<\/li><\/ul>\n\n\n\n<p>EMS personnel\nshould use aerosol-generating procedures (AGPs) <strong><em>only<\/em><\/strong> when they\nwould be life-saving. <\/p>\n\n\n\n<ul><li>Non-rebreather masks, nebulizer treatments, and CPAP generally are to be avoided in this current outbreak <\/li><li>Use the patient\u2019s metered dose inhaler (MDI) with a spacer instead of a nebulizer when available.&nbsp; Bring all such equipment to the hospital with the patient.<\/li><li>Remember that you can place a surgical mask on the patient over oxygen equipment throughout the call.<\/li><li>When AGPs such as CPR, CPAP, nebulizers, and intubation are needed, if feasible, perform those in an open-air environment, or with vehicle windows open while in transit.<\/li><li>During transport, vehicle ventilation in both compartments should be on non-recirculated mode to maximize air changes that reduce potentially infectious particles in the vehicle. <\/li><li>If the vehicle has a rear exhaust fan, use it to draw air away from the cab, toward the patient-care area, and out the back end of the vehicle.<\/li><li><strong><em>Remove all&nbsp;aerosolizing devices from the patient just before entering the      emergency department.<\/em><\/strong> &nbsp;Doing so will help prevent the spread of aerosolized viral particles throughout the ED. <\/li><li><strong><em>Always notify the hospital prior to arrival on any suspect COVID-19 case.<\/em><\/strong> <\/li><\/ul>\n\n\n\n<p>During this\nPandemic Response, EMS personnel are to approach patients at as distance,\ndetermine potential infectious components, assess PPE <strong><em>needs<\/em><\/strong>, and\nhave patients don surgical masks from a distance when symptoms present.&nbsp;\nEMS personnel <strong>must<\/strong> conserve PPE and use only as necessary.&nbsp; N-95\nmasks <strong>must<\/strong> be\nsaved for aerosol producing procedures. &nbsp;<\/p>\n\n\n\n<p><strong>PPE conservation\nmust begin now!&nbsp; Tomorrow is too late.<\/strong>&nbsp; There will be much more severe shortages in the future.&nbsp;\nSurgical masks, gloves, and <em>possibly<\/em> face shields or goggles are\nappropriate for most infectious patients.&nbsp;&nbsp; Plan to clean and reuse\nfollowing CDC guidelines wherever possible.&nbsp; Expect expanded guidance and\nlimitations soon.&nbsp; ALL PPE is coming into shortage, and this will become\nworse.<\/p>\n\n\n\n<p>Exercise droplet protection\nwhen necessary with coveralls or gowns.&nbsp; Uniforms should be laundered\nfrequently during the shift following the transport of potentially contagious\npatients.&nbsp; Thorough decontamination of the cab and patient compartments\nshould be performed after each transport to protect crews and patients.<\/p>\n\n\n\n<p>Consider using\nmaterials such as crash wrap to isolate the driver\u2019s compartment to reduce the\nneed for PPE for drivers.<\/p>\n\n\n\n<p>EMS agencies should consider implementing pre- and post-shift screening for all personnel as recommended in previous guidelines.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Emergency medical services are a component of critical infrastructure.
