COVID Points for EMS

  • Aerosol-generating procedures (AGPs) can be lifesaving.  They also present risks to everyone in the vicinity when the patient has a highly contagious disease.
  • AGPs include CPAP, CPR, nebulizers, intubation, Bi-PAP, suctioning, and high-flow O2 by mask.
    • When using a NRM or cannula, place a mask on the patient over the O2 apparatus.
    • If using a BVM, consider placing a towel on the airway to contain aerosol.
  • Yes, you need to conserve PPE. But if you’re performing an AGP, you and anyone in the back of the ambulance need to be in N95s or PAPRs.
  • Always call the ED for every suspected COVID patient!
    • Make sure you follow the receiving facility’s entry instructions. Hospital personnel may choose to meet you on the ramp to mitigate AGPs; as an example, the may add a HEPA filter between an ETT or trach tube and the BVM.
  • All AGPs require notification, even if you’re transporting from a facility who says that they contacted the receiving hospital. 
    • Notify the hospital about AGPs whether you suspect COVID or not.
  • When you arrive at the hospital, AGPs need to stop during the brief transit from the ED entrance to the room so we aren’t spraying the air with aerosols generated by the procedure. 
    • The exception is CPR (don’t stop that!), but even bagging can be stopped for the 30 seconds or so until you get into a room where hospital personnel are in PPE.

FBI Warning for ‘Zoom-Bombings’ on Online Video Meeting Apps

Zoom has been a godsend to the US economy, giving people the ability to quickly set up massive video conferences in just a few minutes. Unfortunately a quirk of the default configuration allows participants to hijack the conference and share their screens with the entire group. Now the FBI’s Boston field office has issued a new warning after hijackers infiltrated two school-held Zoom sessions to harass a teacher and display the Swastika sign. Quarantine Station Support Plan

This is version 1 of the Quarantine Station Support Plan.  This plan is intended to be a guidance document on setting up potential quarantine sites here in Ohio.  This plan was developed in partnership by ESF6 (Mass Care), ESF8 (Public Health) and various partners. 

The Quarantine Station Support Plan was developed to support the provision of guidance to local jurisdictions for the establishment of a comprehensive quarantine station designed to provide essential services. Quarantine stations will be utilized to limit the spread of COVID-19 within communities, and are not intended to support the relocation of long-term care facilities or hospitals, although these efforts may be supported by some of the strategies detailed herein.

This is a living document, and as the situation changes there will be updates.

Please see the attached report.  Of note, as of today the U.S. has nearly double the amount of cases at that which was officially reported by China.  That being said our mortality is holding at roughly 1/3 theirs.

In addition, at request of Dr. Marriott I have added two additional slides.

The first (slide 16) is a breakdown of call types and crew impressions.  The call types (in green) are by dispatch nature each day for incident types that could been seen with a potential COVID-19 patient.  The crew impression (in red) are those calls in which our crews either put potential COVID-19 as a primary or secondary impression, or filled out the Outbreaks Screening form.

The second new slide (17) is tracking of non-traumatic field termination patients.  This will now be tracked to look for trends of increasing mortality being seen in the field.  Of note, this is currently trending up, and as such I will monitor.  If a more detailed inspection of potential causes is desired please let me know.
